I consent to my child traveling on a club sanctioned activity. 

I understand that coaches will be present and supervising to the best of their abilities and that team players travel having reminded themselves of our club codes of conduct which were a cepted during registration.

I have made the relevant coach aware of any special requirements for this particular trip.
